Keep a list and be prepared to provide the following information during an emergency:

• type of animal
• breed, age, m/f?
• last known or approxiamte weight
• spayed/neutered?
• vaccinations up to date?
• current or prior medical history
• known allergies
• current medications
• current veterinarian
